Residential aged care application for re-accreditation
The Quality Agency's application for re-accreditation (Word) is used by approved providers when applying for re-accreditation. Please access your two-page re-accreditation application (Word). This is a 'word' document that may be completed and submitted via email or printed for hard copy submission.
It's up to the home to decide how to undertake their self-assessment and if they wish to provide the self-assessment information to us at the time of application for re-accreditation or to assessors during the audit. Use our Residential aged care self-assessment tool (Word).
Home Care Quality Review
Home care services undergo a quality review at least every three years. There is no application process for quality reviews. We will contact providers about planning a quality review.
A home care service is required to undertake a self-assessment. It’s up to the service to decide how to undertake their self-assessment.
National Aboriginal and Torres Strait Islander Flexible Aged Care Program Quality Review
As part of the Quality framework, National Aboriginal and Torres Strait Islander services undergo a quality review process in a two year cycle.
As part of this process, services submit a self- assessment of their performance against the quality standards. It’s up to the service to decide how to undertake their self-assessment.
